Sample Insurance Certificates

Below are three sample insurance certificates for installers to view: 

Sample (A) – Projects greater than 50kW 
 

Sample (B) --  Projects 50kW and less

Sample (C) -- Projects 50kW and less with personal auto
 

Applicant/Primary Installers must maintain insurance compliant with Attachment C (Minimum Insurance Requirements).  

Expedited Installers should submit updated insurance certificates to Ebix BPO, our insurance tracking partner.
